【摘要】：國(guó)家的“新醫(yī)改”把大學(xué)生也納入城鎮(zhèn)居民基本醫(yī)療保險(xiǎn)中來(lái)，不僅可以擴(kuò)大社會(huì)互助共濟(jì)的范圍，提高大學(xué)生的醫(yī)療保障水平，也可以減輕學(xué)生家庭因病造成的經(jīng)濟(jì)負(fù)擔(dān)，利于體現(xiàn)社會(huì)公平。在政府的干預(yù)之下，成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度已建立并運(yùn)行起來(lái)，但從大學(xué)生基本醫(yī)療保險(xiǎn)制度推進(jìn)以來(lái)的實(shí)踐表明，現(xiàn)行的大學(xué)生基本醫(yī)療保險(xiǎn)制度設(shè)計(jì)及相關(guān)配套措施沒(méi)有切實(shí)解決對(duì)醫(yī)療服務(wù)供求雙方的行為約束問(wèn)題，致使道德風(fēng)險(xiǎn)嚴(yán)重，大學(xué)生基本醫(yī)療保險(xiǎn)制度運(yùn)行績(jī)效低下。 本文將在認(rèn)清目前成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度的發(fā)展歷程的基礎(chǔ)上，通過(guò)對(duì)成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度實(shí)施狀況的深入分析，找出大學(xué)生基本醫(yī)療保險(xiǎn)制度運(yùn)行中存在的現(xiàn)實(shí)問(wèn)題，借助公共管理學(xué)的相關(guān)理論工具，找出當(dāng)前制度無(wú)效率的根源，為完善成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度提供建議。 本文得出的主要結(jié)論是：政府作為社會(huì)的管理者和公共事業(yè)的組織者，其核心的一個(gè)職能就在于保證公共服務(wù)供給的秩序，保護(hù)公共需求的實(shí)現(xiàn)機(jī)制。（1）從大學(xué)生的實(shí)際保障需求和意愿出發(fā)來(lái)設(shè)計(jì)完善大學(xué)生基本醫(yī)療保險(xiǎn)制度，針對(duì)不同群體的醫(yī)療保障需求，構(gòu)建大學(xué)生社會(huì)醫(yī)療保險(xiǎn)混合體系；（2）加強(qiáng)過(guò)程監(jiān)督，強(qiáng)化政府在大學(xué)生基本醫(yī)療保險(xiǎn)制度中的責(zé)任；（3）明確校醫(yī)院的職責(zé)定位，加速推進(jìn)校醫(yī)院的體制改革；（4）強(qiáng)化宣傳，加強(qiáng)健康教育，，提高大學(xué)生風(fēng)險(xiǎn)防控意識(shí)。 本文在研究?jī)?nèi)容上，將成都市具體的結(jié)構(gòu)性環(huán)境與政策聯(lián)系起來(lái)進(jìn)行系統(tǒng)分析，以成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度實(shí)施情況的問(wèn)卷調(diào)查結(jié)果作為數(shù)據(jù)支撐，深入分析成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度在時(shí)代背景下的具體意義和運(yùn)行結(jié)果，提出完善成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度的建議；在研究方法上，將運(yùn)用公共管理、公共服務(wù)、公共政策執(zhí)行等相關(guān)理論方法，以問(wèn)卷分層抽樣調(diào)查法為主，輔助文獻(xiàn)研究法和實(shí)地觀察法，從公共政策本身、公共政策執(zhí)行對(duì)象、公共政策執(zhí)行環(huán)境等方面，分析影響成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度運(yùn)行的阻礙因素；在研究結(jié)論上，基于成都市具體的環(huán)境和高校學(xué)生的特征分析，結(jié)合國(guó)際國(guó)內(nèi)大學(xué)生保險(xiǎn)制度的成功經(jīng)驗(yàn)，提出了完善成都市大學(xué)生基本醫(yī)療保險(xiǎn)制度的對(duì)策，具有較強(qiáng)的實(shí)踐指導(dǎo)意義。
[Abstract]:The "new medical reform" of the state will also bring college students into the basic medical insurance for urban residents, which can not only expand the scope of social mutual aid, raise the level of medical security for college students, but also alleviate the economic burden caused by the illness of the students' families. Under the intervention of the government, the basic medical insurance system for college students in Chengdu has been established and operated, but the practice since the promotion of the basic medical insurance system for college students shows that, The current design of the basic medical insurance system for college students and related supporting measures have not effectively solved the problem of behavior constraints on both sides of the medical service supply and demand resulting in serious moral hazard and low performance of the basic medical insurance system for college students. Based on a clear understanding of the development of the basic medical insurance system for college students in Chengdu, this paper analyzes the implementation of the basic medical insurance system for college students in Chengdu. This paper finds out the practical problems existing in the operation of the basic medical insurance system for college students, with the help of the relevant theoretical tools of public management, finds out the root causes of the inefficiency of the current system, and provides suggestions for perfecting the basic medical insurance system for college students in Chengdu. The main conclusion of this paper is that the government, as the administrator of the society and the organizer of the public utilities, has the core function of ensuring the order of the public service supply. The realization mechanism of protecting public demand. 1) to design and perfect the basic medical insurance system of college students according to the actual demand and willingness of college students, aiming at the medical security needs of different groups. To build a mixed social medical insurance system for college students and strengthen the process supervision, strengthen the responsibility of the government in the basic medical insurance system for college students and make clear the duty orientation of the hospital, accelerate the institutional reform of the school hospital and strengthen the propaganda. Health education should be strengthened to improve college students' awareness of risk prevention and control. In this paper, the specific structural environment and policy of Chengdu are systematically analyzed, and the results of questionnaire survey on the implementation of the basic medical insurance system for college students in Chengdu are taken as the data support. This paper deeply analyzes the specific significance and operation results of the basic medical insurance system for college students in Chengdu under the background of the times, and puts forward some suggestions for perfecting the basic medical insurance system for college students in Chengdu. Related theoretical methods such as public policy implementation, mainly questionnaire stratified sampling survey, auxiliary literature research and field observation, from the public policy itself, public policy implementation object, public policy implementation environment, etc. Based on the analysis of the specific environment of Chengdu and the characteristics of college students, combined with the successful experience of international and domestic college students' insurance system, this paper analyzes the factors that hinder the operation of the basic medical insurance system for college students in Chengdu. This paper puts forward the countermeasures to perfect the basic medical insurance system for college students in Chengdu, which is of great practical significance.
