【摘要】：目的:應(yīng)用病例對(duì)照研究方法探討血尿酸水平與主動(dòng)脈夾層的相關(guān)性。方法:選取自2012年8月至2014年6月收治的主動(dòng)脈夾層患者160例為病例組,其中Stanford A型AD組58例,Stanford B型AD組102例,選擇同期住院并排除合并AD相關(guān)疾病的157例患者為對(duì)照組。收集患者的一般資料、既往病史、入院時(shí)常規(guī)實(shí)驗(yàn)室檢查,做血尿酸、D-二聚體、超敏CRP水平、血小板計(jì)數(shù)、白細(xì)胞計(jì)數(shù)、血脂分析、血糖。分析主動(dòng)脈夾層的相關(guān)危險(xiǎn)因素。結(jié)果:主動(dòng)脈夾層組和對(duì)照組在年齡分布、性別分布、民族分布方面無(wú)差異,P0.05,兩組在吸煙、飲酒方面相比,差異無(wú)統(tǒng)計(jì)學(xué)意義,P0.05,兩組糖尿病、高血壓、冠心病分布相比,差異無(wú)統(tǒng)計(jì)學(xué)意義,P0.05。主動(dòng)脈夾層組血尿酸水平較對(duì)照組高,(343.08±136.10mmol/l vs.312.5±85.61mmol/l),P=0.017,Stanford A型AD組血尿酸水平高于Stanford B型AD組,(360.31±47.46mmol/l vs.334.28±48.30mmol/l),P=0.012。經(jīng)多因素Logistic逐步回歸分析顯示,高尿酸血癥(OR=1.231,95%CI:0.919~0.981)、高血壓(OR=1.263,95%CI:1.033~2.099)、CRP(OR=1.526,95%CI:1.094~6.129)、D-二聚體(OR=1.012,95%CI:4.004~5.020)為AD的危險(xiǎn)因素。結(jié)論:血尿酸水平異常增高與主動(dòng)脈夾層的發(fā)生具有相關(guān)性。
[Abstract]:Objective: to investigate the correlation between serum uric acid level and aortic dissection.Methods: one hundred and sixty patients with aortic dissection from August 2012 to June 2014 were selected as the case group, including 58 patients with Stanford A AD and 102 patients with Stanford B AD, and 157 patients who were hospitalized in the same period and excluded from AD related diseases were selected as the control group.General data, past medical history, routine laboratory examination on admission, blood uric acid D-dimer, hypersensitive CRP level, platelet count, white blood cell count, blood lipid analysis, and blood sugar were collected.To analyze the risk factors of aortic dissection.Results: there was no significant difference in age distribution, sex distribution and ethnic distribution between aortic dissection group and control group (P 0.05). There was no significant difference in smoking and drinking between the two groups (P 0.05).
