氣管導(dǎo)管生物膜培養(yǎng)對(duì)病原學(xué)檢出率的影響

【摘要】:目的探討機(jī)械通氣危重兒氣管導(dǎo)管細(xì)菌生物膜對(duì)提高病原菌檢出率的作用。方法選擇2013年入住新生兒科需要機(jī)械通氣的新生兒,根據(jù)機(jī)械通氣的時(shí)間將研究對(duì)象分為3組:≤72 h組、73~168 h組及168 h組,以漩渦振蕩器振蕩氣管導(dǎo)管末端,振蕩液培養(yǎng),與同時(shí)送檢的深部痰培養(yǎng)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)比較。結(jié)果在機(jī)械通氣時(shí)間≤72 h組、73~168 h組中,漩渦振蕩器振蕩液與深部痰培養(yǎng)的陽(yáng)性率高度一致,Kappa值分別為0.799、0.710,P值分別為0.000及0.000;在168 h組中,二者中度一致,Kappa值為0.651,P值為0.001。兩種方法分離的菌株以產(chǎn)ESBL酶肺炎克雷伯桿菌最多見(jiàn),產(chǎn)ESBL酶大腸桿菌次之,兩種方法間菌株的構(gòu)成及耐藥性差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論以漩渦振蕩器法獲取氣管導(dǎo)管細(xì)菌生物膜不能提高機(jī)械通氣患兒的病原學(xué)檢測(cè)陽(yáng)性率,對(duì)臨床用藥無(wú)明顯指導(dǎo)意義。
