【摘要】：伴隨國民經(jīng)濟(jì)改革開放30年來的迅猛發(fā)展，中國的社會醫(yī)療保險事業(yè)實現(xiàn)了跨越式的發(fā)展，，取得的成績舉世矚目。但是統(tǒng)籌水平低、城鄉(xiāng)二元分割、制度設(shè)計先天不足等問題已經(jīng)嚴(yán)重影響了基本醫(yī)療保險制度的健康和可持續(xù)發(fā)展，甚至進(jìn)一步加深了區(qū)域、城鄉(xiāng)間社會經(jīng)濟(jì)發(fā)展不均衡的問題�；�于對上述問題的清醒認(rèn)識，統(tǒng)籌城鄉(xiāng)醫(yī)療保險制度成為當(dāng)務(wù)之急并被提上了政府的議事日程。統(tǒng)籌城鄉(xiāng)醫(yī)療保險，可以顯著提高醫(yī)療保險的保障水平，推動醫(yī)療保險制度健康發(fā)展，同時也是當(dāng)今中國建立完善的社會保障制度體系的客觀需要。統(tǒng)籌城鄉(xiāng)醫(yī)療保險制度可以提高政府在社會保障領(lǐng)域的轉(zhuǎn)移支付水平，彌補(bǔ)城鄉(xiāng)、區(qū)域間的經(jīng)濟(jì)發(fā)展鴻溝，促進(jìn)社會經(jīng)濟(jì)全面、協(xié)調(diào)、可持續(xù)發(fā)展，特別是對農(nóng)村人口、弱勢群體在公平地享受社會保障福利上起到巨大的推動作用。統(tǒng)籌城鄉(xiāng)醫(yī)療保險制度工作要融入到發(fā)展和完善中國社會保障體系的建設(shè)中去，在廣泛吸收國內(nèi)外成功的理論成果和實踐經(jīng)驗的基礎(chǔ)上，以中國經(jīng)濟(jì)社會發(fā)展的現(xiàn)實國情為落腳點，謀篇布局，統(tǒng)籌協(xié)調(diào)，全盤考量，逐步改進(jìn)，提升基本醫(yī)療保險統(tǒng)籌層次，優(yōu)化基本醫(yī)療保險服務(wù)質(zhì)量。 論文以統(tǒng)籌城鄉(xiāng)醫(yī)療保險制度的背景和意義為切入點，首先闡述并分析了社會保障、社會保險、城鄉(xiāng)二元經(jīng)濟(jì)結(jié)構(gòu)、城鄉(xiāng)基本醫(yī)療保險一體化等概念，為文章打下了理論基礎(chǔ)。其次概括介紹了中國城鄉(xiāng)基本醫(yī)療保險的發(fā)展現(xiàn)狀，并對存在的問題做了深入剖析，認(rèn)為這些問題影響中國社會經(jīng)濟(jì)健康可持續(xù)發(fā)展，損害城鄉(xiāng)公民平等享有醫(yī)療保障的權(quán)利。再次通過實證分析，以一個醫(yī)保工作者的視角總結(jié)了廣州在統(tǒng)籌城鄉(xiāng)醫(yī)療保險過程中取得的寶貴經(jīng)驗，并借助數(shù)據(jù)分析提出值得中國現(xiàn)行統(tǒng)籌城鄉(xiāng)醫(yī)療保險工作加以借鑒的地方。最后根據(jù)以上分析，為加快推進(jìn)廣州市統(tǒng)籌城鄉(xiāng)醫(yī)療保險制度提出相應(yīng)的目標(biāo)和對策。其中，對于目標(biāo)的實現(xiàn)是分階段的，從統(tǒng)籌工作的進(jìn)展與要求將其分為短期目標(biāo)、中期目標(biāo)和長期目標(biāo)；對策上從完善法制建設(shè)、加強(qiáng)統(tǒng)籌管理、提升統(tǒng)籌質(zhì)量、提高服務(wù)水平等方面展開闡述，力求穩(wěn)步推進(jìn)醫(yī)療保險城鄉(xiāng)統(tǒng)籌工作。
[Abstract]:With the rapid development of national economy reform and opening up in the past 30 years, China's social medical insurance industry has achieved a leapfrog development, and the achievements have attracted worldwide attention. However, the low level of overall planning, the dual division of urban and rural areas, and the inherent deficiencies in the design of the system have seriously affected the health and sustainable development of the basic medical insurance system, and even further deepened the region. The problem of uneven social and economic development between urban and rural areas. Based on the sobering understanding of the above problems, coordinating the urban and rural medical insurance system has become a top priority and has been put on the government's agenda. Coordinating urban and rural medical insurance can significantly improve the level of medical insurance, promote the healthy development of medical insurance system, but also is the objective need of establishing a perfect social security system in China. As a whole, the urban and rural medical insurance system can improve the level of government transfer payments in the field of social security, make up the gap in economic development between urban and rural areas, and promote comprehensive, coordinated and sustainable development of the social economy, especially for the rural population. Vulnerable groups play a huge role in the fair enjoyment of social security benefits. As a whole, the urban and rural medical insurance system should be integrated into the development and improvement of China's social security system, and on the basis of extensive absorption of successful theoretical achievements and practical experience at home and abroad, Taking the reality of China's economic and social development as its foothold, it is necessary to seek for a general layout, overall coordination, overall consideration, gradual improvement, promotion of the overall level of basic medical insurance, and optimization of the quality of basic medical insurance services. Based on the background and significance of urban and rural medical insurance system, this paper firstly expounds and analyzes the concepts of social security, social insurance, urban-rural dual economic structure, and the integration of urban and rural basic medical insurance, which lays a theoretical foundation for the article. Secondly, it introduces the development status of basic medical insurance in urban and rural areas in China, and analyzes the existing problems deeply, and thinks that these problems affect the healthy and sustainable development of social economy in China and impair the equal right of citizens in urban and rural areas to enjoy medical security. Thirdly, through empirical analysis, this paper summarizes the valuable experience of Guangzhou in the process of coordinating urban and rural medical insurance from the perspective of a medical insurance worker. With the help of data analysis, the author points out that the current urban and rural medical insurance in China should be used for reference. Finally, according to the above analysis, the paper puts forward corresponding objectives and countermeasures to accelerate the urban and rural medical insurance system in Guangzhou. Among them, the realization of the goal is phased, according to the progress and requirements of the overall work, it can be divided into short-term goal, medium-term goal and long-term goal, improve the legal system construction, strengthen the overall management, improve the overall planning quality, Improve the level of services and other aspects of the elaboration, and strive to steadily promote medical insurance urban and rural coordination work.
