【摘要】：目的觀察慢性腎臟病(CKD)3-5D期患者血清骨保護素(OPG)變化,并探討和腹主動脈鈣化(AAC)的相關(guān)性。方法選取86例CKD 3-5D期患者為研究對象,根據(jù)腹主動脈有無鈣化分為AAC組和non-AAC組,應(yīng)用ELISA法測定各組患者血清OPG水平;采用腹部側(cè)位片評估AAC程度。結(jié)果 86例CKD患者中AAC 51例(59.3%)。51例ACC患者OPG明顯高于35例non-AAC患者(P0.01)。CKD5期未行腎臟替代治療患者血清OPG顯著高于CKD3-4期患者(P0.05);CKD5D期血清OPG水平和CKD5期未行腎臟替代治療患者OPG差異無統(tǒng)計學(xué)意義。血清OPG與e GFR呈顯著負相關(guān)(r=-0.357,P=0.011),與AAC程度相關(guān)(r=0.258,P=0.017)。Logistic回歸分析發(fā)現(xiàn):男性(β=2.568,P=0.048)、吸煙(β=4.337,P=0.013)、OPG(β=1.273,P=0.003)是影響AAC的主要因素。結(jié)論在CKD患者中,血管鈣化發(fā)生率高,隨著腎功能惡化,血清OPG水平有升高趨勢。男性、吸煙及OPG是AAC的危險因素,可能參與AAC的發(fā)生與發(fā)展。
[Abstract]:Objective to observe the changes of serum osteoprotegerin (OPG) in patients with chronic kidney disease (CKD) 3-5D) and to explore the correlation between serum osteoprotegerin and calcified (AAC) in abdominal aorta. Methods 86 patients with stage 3-5D of CKD were divided into AAC group and non-AAC group according to the calcification of abdominal aorta. The serum OPG level was measured by ELISA method and the degree of AAC was evaluated by abdominal lateral radiography. Results the serum OPG levels of AAC 51 (59.3%) .51 ACC patients with CKD were significantly higher than those of 35 non-AAC patients (P0.01). CKD5 patients without renal replacement therapy (P0.05) and CKD5 patients without renal replacement therapy (CKD5). There was no significant difference in OPG. There was a significant negative correlation between serum OPG and e GFR (r-0.357 P0. 011). Logistic regression analysis showed that: male (尾 -2.568), smoking (尾 4.337), OPG (尾 1.273P0. 003) were the main factors affecting AAC. Conclusion the incidence of vascular calcification is high in patients with CKD, and the serum OPG level tends to increase with the deterioration of renal function. In men, smoking and OPG are risk factors for AAC and may be involved in the occurrence and development of AAC.
