【摘要】：目的:系統(tǒng)評(píng)價(jià)ACEIs和ARBs對(duì)腎移植受者血紅蛋白濃度的影響。方法:計(jì)算機(jī)檢索Medline、EMBase、Cochrane CENTRAL、中國(guó)生物醫(yī)學(xué)數(shù)據(jù)庫(kù)(CBM)、相關(guān)期刊論文(CNKI)、中文科技期刊全文數(shù)據(jù)庫(kù)(VIP)和萬(wàn)方數(shù)據(jù)庫(kù),檢索截止至2016年7月,納入腎移植受者接受ACEIs或ARBs治療與安慰劑或空白對(duì)照的隨機(jī)對(duì)照試驗(yàn)(RCTs),評(píng)價(jià)納入RCTs的偏倚風(fēng)險(xiǎn),對(duì)同質(zhì)性結(jié)果進(jìn)行Meta分析,并評(píng)價(jià)結(jié)果的GRADE證據(jù)質(zhì)量等級(jí)。結(jié)果:共納入10個(gè)RCTs,受試者1503例。Meta分析結(jié)果顯示隨訪3月、6月、12月及12月以上,ACEIs或ARBs組血紅蛋白濃度均低于安慰劑或空白對(duì)照組,MD、95%CI和P值分別為:SMD=-1.50,95%CI(-2.10,-0.90);SMD=-0.40,95%CI(-0.77,-0.02),P=0.04;SMD=-0.79,95%CI(-1.20,-0.38),P=0.000 1;SMD=-0.72,95%CI(-0.84,-0.60),P0.000 01。4個(gè)結(jié)果的GRADE證據(jù)質(zhì)量等級(jí)分別為:中、低、低、中等質(zhì)量。結(jié)論:基于當(dāng)前證據(jù),RAS抑制劑可降低腎移植受者的血紅蛋白濃度。
