文章摘要：目的通過采用基于患者報(bào)告臨床結(jié)局的方式觀察針刺聯(lián)合西藥抗抑郁的療效。方法將88例輕、中度抑郁癥患者隨機(jī)分為電針組28例，手針組25例，藥物組35例。藥物組患者給予鹽酸帕羅西汀片，治療第1、2天每日10mg，每日1次，從第3天開始劑量增加到每日20mg，每日1次，共服用6用。手針組在藥物組基礎(chǔ)上配合針刺治療，主穴：百會(huì)、印堂，基本配穴：風(fēng)府、風(fēng)池（雙側(cè)）、大椎、內(nèi)關(guān)（雙側(cè)）、三陰交（雙側(cè)）；電針組在藥物組基礎(chǔ)上配合電針治療，選穴同手針組，以百會(huì)和印堂、雙側(cè)風(fēng)池連接于電針儀正負(fù)極，刺激頻率選2／15Hz疏密交替波。手針組和電針組均隔天治療1次，每次針刺30min，1周治療3次，療程6周。觀察各組治療前后漢密爾頓抑郁量表17項(xiàng)（HAMD-17）、基于患者報(bào)告的癥狀量表（MYMOP）評(píng)分變化情況，治療后評(píng)價(jià)臨床療效。結(jié)果電針組臨床療效總有效率為89．28％、手針組為92．00％、藥物組為85．71％，電針組和手針組優(yōu)于藥物組（P〈0．05）。各組患者治療后MYMOP各項(xiàng)評(píng)分與本組治療前比較差異均有統(tǒng)計(jì)學(xué)意義（P〈0．05），，并且電針組和手針組各項(xiàng)評(píng)分明顯低于藥物組（P〈0．05），而電針組和手針組各項(xiàng)評(píng)分比較差異無統(tǒng)計(jì)學(xué)意義（P〉0．05）。結(jié)論針刺聯(lián)合西藥可以顯著降低輕、中度抑郁癥患者HAMD．17及MYMOP評(píng)分，改善主要癥狀、總體健康狀況和活動(dòng)情況，優(yōu)于單純西藥治療。

Abstr：Objective To observe the therapeutic effect of acupuncture combined with western medicine for depression based on patient-reported outcomes （PROs）. Methods Totally 88 cases of mild to moderate depression were randomized into the electro-acupuncture （EA） group （28 cases）, manual acupuncture group （25 cases）, and western medicine group （35 cases）. The western medicine group was given Paroxetine Hydrochloride Tablets, 10 mg/ d during the first two days and 20 mg/d from the 3rd day of treatment for a total of six weeks. The manual acupuncture group and EA group were added with acupuncture or EA on the main acupoints of Baihui （ GV20 ） and Yintang （ EX- HN3） and basic acupoints of Fengfu （GV16）, Fengchi （GB20, bilateral）, Dazhui （GV14）, Neiguan （PC6, bilateral） and Sanyinjiao （ SP6, bilateral） on the basis of western medicine group. The manual acupuncture group and EA group were given acupuncture 30 min each time and once every other day for six weeks. The therapeutic effect was evaluated according to the Hamilton Rating Scale for Depression-17 items （HAMD-17） and the Measure Yourself Medical Outcome Profile （MYMOP）. Results The total effective rate of the EA group, manual acupuncture group and western medicine group were 89.28%, 92.00% and 85.71% respectively. The total effective rates of the EA group and manual acupuncture group were superior to that of the western medicine group. There were significant differences between before and after treatment in the scores of MYMOP in all groups （P 〈 O. 05 ）. The scores of MY- MOP in the EA group and manual acupuncture group were significantly lower than those in the western medicine group after treatment （ P 〈 0.05）.
