【摘要】：在人們生活水平不斷提高的大背景下,人們的衛(wèi)生服務(wù)需求和衛(wèi)生費(fèi)用支出呈現(xiàn)逐年上漲的趨勢(shì),與之相應(yīng),醫(yī)療保險(xiǎn)基金支付壓力越來越大。因此如何選擇醫(yī)療保險(xiǎn)支付方式和在控制衛(wèi)生費(fèi)用的同時(shí)激勵(lì)衛(wèi)生服務(wù)機(jī)構(gòu)提高服務(wù)質(zhì)量,成為現(xiàn)階段研究的主要問題。而支付方式改革涉及多方利益,良好的支付方式設(shè)計(jì)有利于控制醫(yī)療費(fèi)用的不合理增長(zhǎng),節(jié)約醫(yī)療資源,提高新型農(nóng)村合作醫(yī)療基金使用效率,提高參合農(nóng)民的受益水平。本文借鑒我國(guó)醫(yī)療保險(xiǎn)住院支付方式改革的主要經(jīng)驗(yàn)與做法,對(duì)比不同支付方式的優(yōu)勢(shì)和弊端,并分析我國(guó)新農(nóng)合支付方式改革現(xiàn)狀,評(píng)價(jià)現(xiàn)有支付方式改革的成效,為進(jìn)一步探索支付方式改革路徑提供可靠依據(jù),從而解決新農(nóng)合運(yùn)行過程中存在的醫(yī)療費(fèi)用增長(zhǎng)較快的問題。針對(duì)新農(nóng)合的支付方式改革,本研究進(jìn)一步從利益相關(guān)者的角度進(jìn)行分析,包括對(duì)管理機(jī)構(gòu)的監(jiān)管、對(duì)定點(diǎn)醫(yī)療機(jī)構(gòu)的監(jiān)管和對(duì)參合農(nóng)民的監(jiān)管三大部分。以唐山市豐潤(rùn)區(qū)新農(nóng)合支付中的監(jiān)管為例,對(duì)新農(nóng)合不同利益相關(guān)者的監(jiān)管狀況進(jìn)行了分析,對(duì)相關(guān)監(jiān)管方法進(jìn)行探討,目的是通過對(duì)支付中監(jiān)管有關(guān)問題的研究,以期發(fā)展和創(chuàng)新新農(nóng)合支付監(jiān)管的方式方法,為新農(nóng)合支付監(jiān)管工作提供參考。
[Abstract]:Under the background of the continuous improvement of people's living standard, the demand for health services and the expenditure of health expenses are increasing year by year. Accordingly, the pressure of medical insurance fund to pay is increasing. Therefore, how to choose the payment method of medical insurance and how to encourage health service organizations to improve the service quality while controlling the health expenses become the main problems in the current research. The reform of payment mode involves various interests. A good design of payment mode is conducive to controlling the unreasonable growth of medical expenses, saving medical resources, improving the efficiency of using the new rural cooperative medical fund, and improving the benefit level of participating farmers. This article draws lessons from the main experience and practice of medical insurance inpatient payment reform in China, compares the advantages and disadvantages of different payment methods, analyzes the current situation of the reform of the new rural cooperative payment mode in China, and evaluates the effectiveness of the existing payment mode reform. It provides a reliable basis for further exploring the path of payment reform, thus solving the problem of rapid increase of medical expenses in the course of operation of NCMS. In view of the reform of payment mode of NCMS, this study further analyzes from the perspective of stakeholders, including the supervision of management agencies, the supervision of designated medical institutions and the supervision of participating farmers. Taking the supervision of the new rural cooperative payment in Fengrun District of Tangshan City as an example, this paper analyzes the supervision situation of the different stakeholders of the new rural cooperation, and probes into the relevant supervision methods, the purpose of which is to study the relevant problems in the supervision of the payment. In order to develop and innovate the new rural cooperative payment supervision methods and provide a reference for the new rural cooperative payment supervision.
