【摘要】：目的:評(píng)價(jià)鼻內(nèi)鏡輔助下低溫等離子(endoscopic assisted low temperature plasma,ELTP)與電動(dòng)吸割器(electric planer,EP)治療腺樣體肥大療效的差異。方法:計(jì)算機(jī)檢索中國知網(wǎng),萬方數(shù)據(jù)庫,維普數(shù)據(jù)庫,中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫,PubMed,Embase和Cochrane數(shù)據(jù)庫,查找有關(guān)ELTP與EP切除腺樣體療效對(duì)比的隨機(jī)對(duì)照研究。檢索時(shí)限為2007年1月至2016年6月。對(duì)納入的文獻(xiàn)進(jìn)行方法學(xué)評(píng)價(jià),并用Revman 5.2軟件對(duì)符合標(biāo)準(zhǔn)的研究進(jìn)行Meta分析。結(jié)果:最終納入13個(gè)隨機(jī)對(duì)照研究,共1 448名患者,其中722名患者接受ELTP手術(shù),即ELTP組,726名患者接受EP手術(shù),即EP組。Meta分析結(jié)果示:與EP手術(shù)相比,ELTP手術(shù)的總體有效率明顯提高(OR=3.19,95%CI:1.42~7.15),P=0.005],并且術(shù)中出血量明顯減少(MD= 20.35,95%CI: 20.84~ 19.87,P0.001),手術(shù)時(shí)間明顯縮短(MD= 15.71,95%CI: 18.06~ 12.17,P0.001),并發(fā)癥的發(fā)生率也明顯降低(OR=0.13,95%CI:0.06~0.30,P0.001);而兩組術(shù)中圓枕損傷發(fā)生率、術(shù)后腺樣體殘留及術(shù)后出血發(fā)生率差異無統(tǒng)計(jì)學(xué)意義。結(jié)論:相比于EP,ELTP在腺樣體肥大手術(shù)中更具有優(yōu)勢。
[Abstract]:Objective: to evaluate the difference between endoscopic low temperature plasma (endoscopic assisted low temperature plasma,ELTP) and electric aspiration (electric planer,EP) in the treatment of adenoid hypertrophy. Methods: a randomized controlled study on the comparison of the effects of adenoidectomy with ELTP and EP was carried out by computer searching the database of Chinese Zhiwang, Wanfang database, Weip database, Chinese biomedical literature database, PubMed,Embase database and Cochrane database. The time limit for retrieval is from January 2007 to June 2016. The methodological evaluation of the included literature was carried out, and the Meta analysis of the compliant research was carried out with Revman 5.2 software. Results: a total of 1 448 patients were enrolled in 13 randomized controlled trials, of which 722 patients underwent ELTP operation, that is, ELTP group, and 726 patients underwent EP operation, that is, EP group. The results of Meta analysis showed that compared with EP operation, 726 patients underwent EP operation. The overall effective rate of ELTP operation was significantly improved (OR=3.19,95%CI:1.42~7.15, P0. 005), and the amount of intraoperative bleeding was decreased (MD= 20. 35% 95 CI: 20. 84 ~ 19. 87% P0.001). The operative time was significantly shortened (MD= 15.71 / 95 CI: 18.06 ~ 12.17 / P0.001), and the incidence of complications (OR=0.13,95%CI:0.06~0.30,P0.001) was also significantly reduced (P < 0.05). There was no significant difference between the two groups in the incidence of occipital injury, postoperative adenoid residue and postoperative hemorrhage. Conclusion: compared with EP,ELTP, it has more advantages in adenoid hypertrophy surgery.
