【摘要】：目的:探討中頻穴位電刺激聯(lián)合他莫昔芬片治療少弱精子癥的臨床效果及其可行性與安全性。方法:120例少弱精子癥患者隨機(jī)分為他莫昔芬片口服治療組(他莫昔芬組)、中頻穴位電刺激治療組(穴位電刺激組)與聯(lián)合治療組,每組40例。連續(xù)治療8周,記錄各組治療前后的精液體積、精子總數(shù)及濃度、總活力、前向運(yùn)動(dòng)(PR)精子百分率及血清生殖激素(FSH、LH、T)水平,對(duì)比分析3組患者治療前后的精液參數(shù)及生殖激素水平。結(jié)果:所有患者均完成本研究,治療前后3組精液體積無(wú)明顯改善(P0.05)。與治療前對(duì)比,3組均可改善生殖激素(FSH、LH、T)水平(P0.05);他莫昔芬組可以顯著提高精子總數(shù)[(42.65±5.78)×10~6vs(25.16±2.05)×10~6,P0.05]及精子濃度[24.31±2.59)×10~6/ml vs(12.15±2.51)×10~6/ml,P0.05],而精子總活力及PR精子百分率無(wú)明顯改善[(22.61±5.75)%vs(21.78±8.81)%;(16.76±5.86)%vs(15.87±7.81)%,P均0.05];穴位電刺激組可以增加精子總活力率[(32.43±4.97)%vs(24.81±8.27)%,P0.05]及PR精子百分率[(27.17±5.09)%vs(19.71±9.15)%,P0.05],而精子總數(shù)及精子濃度無(wú)明顯改善[(25.87±4.96)×10~6vs(23.23±3.14)×10~6;(14.12±2.47)×10~6/ml vs(11.27±2.24)×10~6/ml,P均0.05];聯(lián)合治療組不僅可以提高精子總數(shù)[(51.14±3.69)×10~6vs(26.17±5.05)×10~6,P0.05]及精子濃度[(27.28±1.98)×10~6/ml vs(12.78±2.41)×10~6/ml,P0.05],而且可以顯著提高精子總活力[(37.12±5.33)%vs(23.89±9.05)%,P0.05]及PR精子百分率[(31.09±7.12)%vs(17.14±8.04)%,P0.05],并且顯著優(yōu)于他莫昔芬組和穴位電刺激組(P0.05)。聯(lián)合治療組總有效率(97.5%)顯著高于他莫昔芬組(72.5%)和穴位電刺激組(75.0%),P0.05。結(jié)論:中頻穴位電刺激聯(lián)合他莫昔芬片口服可以明顯改善特發(fā)性少弱精子癥患者的精液質(zhì)量和生殖激素水平,有一定的臨床推廣意義。
[Abstract]:Objective: to investigate the clinical efficacy and safety of middle frequency acupoint electric stimulation combined with tamoxifen tablets in the treatment of oligozoospermia. Methods 120 cases of oligoasthenospermia were randomly divided into three groups: tamoxifen oral treatment group (tamoxifen group), medium frequency acupoint stimulation group (Acupoint electric stimulation group) and combined treatment group (40 cases in each group). After 8 weeks of continuous treatment, semen volume, total sperm count and concentration, total motility, forward motility (PR) sperm percentage and serum reproductive hormone (FSH,LH,T) levels were recorded before and after treatment. Semen parameters and reproductive hormone levels were analyzed before and after treatment.
