【摘要】：目的：本研究通過收集陰道-閉孔由內(nèi)向外尿道無張力懸吊術(shù)（TVT-O）治療女性壓力性尿失禁（SUI）患者的臨床資料，隨訪并評估患者術(shù)后的療效、安全性、生活質(zhì)量、性生活及其性伴侶性生活情況的變化。 方法：對2008年至2013年在我院行TVT-O的144例壓力性尿失禁患者及其性伴侶，，采用國際尿失禁咨詢委員會尿失禁問卷簡表、癥狀自評量表、國際女性性功能量表、國際男性勃起功能指數(shù)簡表5項，分別對手術(shù)前后情況進行評分并行統(tǒng)計學分析。 結(jié)果：1.TVT-O術(shù)有效率高達98.6%。術(shù)后并發(fā)癥發(fā)生率均非常低。2.患者手術(shù)前后心理情況的改善有顯著統(tǒng)計學意義（P＜0.05）。其中人際關(guān)系改善顯著（P＜0.01）。 3.144例患者中有52例(36.1%)術(shù)前、術(shù)后均無性生活，剩余92例(63.9%)術(shù)前有性生活者術(shù)后未停止性生活，且性生活滿意度有所提高，有統(tǒng)計學意義（p＜0.05）。其余指標手術(shù)前后未發(fā)現(xiàn)有統(tǒng)計學差異(P0.05)。4.92例患者性伴侶的性生活滿意度在術(shù)后有提高，有統(tǒng)計學意義（p＜0.05）。其余指標手術(shù)前后無統(tǒng)計學差異（p0.05） 結(jié)論：1. TVT-O術(shù)對于治療SUI是一種安全有效的手術(shù)方式。 2. TVT-O術(shù)同時能有效改善患者因SUI所致的心理障礙，提高患者術(shù)后生活質(zhì)量。 3. TVT-O術(shù)不會對患者或其性伴侶的性生活質(zhì)量產(chǎn)生負面的影響，性生活中不自主漏尿癥狀的好轉(zhuǎn)可能會提高雙方對性生活的滿意度。
[Abstract]:Objective: to collect the clinical data of female patients with stress urinary incontinence (SUI) treated by vagina-obturator tension-free suspension (TVT-O) from the inside to the outside urethra, and to evaluate the effect, safety and quality of life of the patients after operation. Changes in sexual life and sexual life of sexual partners. Methods: a total of 144 patients with stress urinary incontinence and their sexual partners who underwent TVT-O in our hospital from 2008 to 2013 were investigated by using the International urinary Incontinence Advisory Committee's urinary incontinence questionnaire, the symptom Checklist and the International female sexual function scale. Five items of international male erectile function index were evaluated before and after operation and were analyzed statistically. Results: 1. The effective rate of TVT-O operation was 98.6%. The incidence of postoperative complications was very low. The improvement of psychological state before and after operation was statistically significant (P < 0.05). Among the 3.144 patients, 52 cases (36.1%) had no sexual life before operation, and the remaining 92 cases (63.9%) had no sexual life after operation. There was statistical significance (p < 0.05). There was no significant difference before and after operation (P0.05) .4.92 patients' sexual life satisfaction increased after operation (p < 0.05). There was no statistical difference between the other indexes before and after operation (p0.05) conclusion: 1. TVT-O is a safe and effective procedure for the treatment of SUI. 2. TVT-O can also effectively improve the psychological disorders caused by SUI, and improve the quality of life of the patients after operation. TVT-O has no negative effect on the quality of sexual life of patients or their sexual partners. The improvement of involuntary urine leakage in sexual life may increase the satisfaction of both parties.
