幽門(mén)螺桿菌感染的家庭聚集現(xiàn)象

【摘要】:幽門(mén)螺桿菌(H.pylori)是一種導(dǎo)致全球近50%人群感染的微需氧菌,是慢性胃炎、消化性潰瘍和胃粘膜相關(guān)淋巴組織淋巴瘤的主要致病因子,并且與胃癌的發(fā)生密切相關(guān),1994年國(guó)際癌癥研究機(jī)構(gòu)將其列為Ⅰ類致癌因子[1]。根除H.pylori可有效地防治其相關(guān)性疾病尤其是胃癌的發(fā)生,F(xiàn)已明確H.pylori相關(guān)性疾病是一種感染性疾病,它可在人與人之間傳播[2-4],家庭是H.pylori最易傳播的場(chǎng)所,大量研究提示H.pylori感染存在家庭聚集性,與H.pylori根除治療失敗、復(fù)發(fā)和再感染密切相關(guān),并且在反復(fù)治療復(fù)發(fā)與再感染病例過(guò)程中,會(huì)誘導(dǎo)耐藥菌株的產(chǎn)生,增加根除難度。本文就H.pylori感染家庭聚集現(xiàn)象、其產(chǎn)生機(jī)制及如何防治等進(jìn)行一綜述。
[Abstract]:Helicobacter pylori (H.pylori) is a microaerobic bacterium that causes infection in nearly 50% of the world's population. It is the main pathogenic factor of chronic gastritis, peptic ulcer and gastric mucosa-associated lymphoid tissue lymphoma, and is closely related to the occurrence of gastric cancer. In 1994, the International Agency for Research on Cancer (IARC) classified it as a class I carcinogen [1]. Eradication of H.pylori can effectively prevent and treat its associated diseases, especially gastric cancer. It has been established that H.pylori related disease is an infectious disease that can be transmitted from person to person [2-4]. Family is the most easily transmitted site of H.pylori. A large number of studies suggest that H.pylori infection has family aggregation. It is closely related to the failure of H.pylori eradication treatment, recurrence and re-infection, and in the process of repeated treatment of recurrent and re-infection cases, it will induce the production of drug-resistant strains and increase the difficulty of eradication. This article reviews the phenomenon of family aggregation of H.pylori infection, its producing mechanism and how to prevent and cure it.
