【摘要】：目的探討中老年慢性胃病患者幽門螺桿菌(Hp)感染與不同胃病、胃黏膜病理改變之間的關(guān)系。方法對(duì)631例50歲以上的慢性胃病患者行胃鏡、病理活檢和~(13)C-尿素呼氣試驗(yàn)(~(13)C-UBT),觀察不同年齡組、不同胃病組Hp感染情況,觀察胃黏膜不同病理組織學(xué)改變情況。結(jié)果中老年慢性胃病患者HP感染率為61.17%。50~59歲、60~69歲、70~79歲、≥80歲組Hp感染率分別為64.26%、64.44%、58.42%、48.28%;慢性淺表性胃炎、慢性萎縮性胃炎、胃潰瘍、十二指腸潰瘍和胃癌(GC)Hp感染率分別為41.38%、60.08%、72.19%、74.23%和36.84%,消化性潰瘍Hp感染率最高;胃黏膜炎癥、腸上皮化生、低級(jí)別上皮內(nèi)瘤變、高級(jí)別上皮內(nèi)瘤變、GC的Hp感染率分別為52.43%、76.62%、55.04%、47.62%、36.84%,腸上皮化生組Hp感染率明顯高于其他組,隨著病變程度加深Hp感染率逐漸下降。結(jié)論中老年慢性胃病與HP感染密切相關(guān),隨著年齡增長(zhǎng),HP感染率呈下降趨勢(shì)。HP感染在胃黏膜炎癥、腸上皮化生、不典型增生、GC病理學(xué)改變、進(jìn)展中起促進(jìn)作用,應(yīng)早期根除Hp。
[Abstract]:Objective to investigate the relationship between Helicobacter pylori (Hp) infection and pathological changes of gastric mucosa in middle-aged and aged patients with chronic gastric disease. Methods 631 patients over 50 years old with chronic gastropathy underwent gastroscopy, pathological biopsy and ~ (13) C-urea breath test (~ (13) C-UBT).
