【摘要】：正患者男,45歲。因黑便、乏力20 d入院。20 d前,患者出現(xiàn)黑便,全身乏力,無嘔吐。查體:P 72次/min,BP 100/62 mm Hg,神志清楚;心肺未見異常,腹壁靜脈無曲張,肝脾肋下未捫及,移動性濁音陰性。輔助檢查:血常規(guī)Hb 91 g/L,乙肝表面抗原陰性,大便隱血陽性。胃鏡:胃底靜脈曲張,慢性淺表性胃竇炎。腸鏡:橫結(jié)腸見蛇形藍色靜脈曲張,直腸正常。腹部彩超:肝臟鈣化灶,脾臟增大。上腹部增強MRI:脾大,脾靜脈遠端顯示不清,周圍大量側(cè)支循環(huán)形成,多系發(fā)育畸形,伴胃底周圍靜脈曲張,
[Abstract]:The patient is 45 years old. The patient was admitted to hospital for 20 days due to melanosis. 20 days before, the patient developed black stool, general fatigue and no vomiting. Body examination: P 72 times / min,BP 100 / 62 mm Hg, consciousness was clear, heart and lung were not abnormal, abdominal wall vein was not varicose, liver and spleen were not palpable, moving turbidity was negative. Auxiliary examination: blood routine Hb 91 g / L, hepatitis B surface antigen negative, stool occult blood positive. Gastroscopy: varicose gastric fundus, chronic superficial sinusitis. Colonoscopy: serpentine blue varices are seen in the transverse colon and rectum is normal. Abdominal color Doppler ultrasound: liver calcification focus, spleen enlargement. The splenomegaly of MRI: was enhanced by epigastric enhancement, the distal splenic vein was not clearly displayed, a large number of collateral circulation formed around it, multiple lineage deformity and varicose veins around the gastric fundus were observed.
