【摘要】：目的比較并探究奧曲肽聯(lián)合不同劑量垂體后葉素治療血吸蟲性肝硬化繼發(fā)上消化道出血的應(yīng)用研究。方法106例消化科住院并進(jìn)行治療的血吸蟲性肝硬化繼發(fā)性上消化道出血的患者,隨機(jī)分為對照組和觀察組,各53例。對照組患者使用奧曲肽0.05mg/ml進(jìn)行前期靜脈推注,保持25μg/h維持靜脈滴注的效果,保持0.5ml/min進(jìn)行維持治療。觀察組患者奧曲肽聯(lián)合垂體后葉素1U/ml靜脈滴注0.5h左右,保持0.5ml/min進(jìn)行維持治療。比較兩組患者的臨床效果、止血效果以及不良反應(yīng)發(fā)生的情況。結(jié)果兩組患者總有效率比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);兩組的平均止血時(shí)間和止血量比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05);觀察組的不良反應(yīng)發(fā)生率明顯小于對照組(P0.05)。結(jié)論奧曲肽聯(lián)合小劑量垂體后葉素治療血吸蟲性肝硬化繼發(fā)上消化道出血不良反應(yīng)較少,用藥量較少,可在臨床上進(jìn)一步推廣運(yùn)用。
[Abstract]:Objective to compare and investigate the clinical application of octreotide combined with different doses of pituitrin in the treatment of upper gastrointestinal hemorrhage secondary to schistosomiasis cirrhosis. Methods 106 patients with secondary upper gastrointestinal bleeding due to schistosomiasis cirrhosis were randomly divided into control group and observation group with 53 cases each. Patients in the control group were treated with octreotide 0.05mg/ml to maintain the effect of intravenous infusion at 25 渭 g / h and maintenance therapy with 0.5ml/min. Patients in the observation group were treated with octreotide combined with pituitrin 1U/ml intravenous drip for 0.5 h to maintain 0.5ml/min. The clinical effect, hemostatic effect and adverse reaction were compared between the two groups. Results there was no significant difference in the total effective rate between the two groups (P 0.05), the mean hemostasis time and hemostasis volume were not significantly different between the two groups, and the incidence of adverse reactions in the observation group was significantly lower than that in the control group (P 0.05). Conclusion Octreotide combined with low dose of pituitrin in the treatment of secondary upper gastrointestinal hemorrhage due to schistosomiasis cirrhosis has fewer adverse reactions and less dosage, so it can be popularized in clinical practice.
