文章摘要：目的 探討醫(yī)護合作決策分級護理級別在婦產(chǎn)科臨床護理工作中的應(yīng)用效果.方法 將婦產(chǎn)科二區(qū)設(shè)為常規(guī)組,常規(guī)組按照傳統(tǒng)的護理分級模式,由醫(yī)生按照患者實際病情的輕、重、緩、急和分級護理標(biāo)準(zhǔn)決定護理級別,護士按醫(yī)囑執(zhí)行；婦產(chǎn)科一區(qū)設(shè)為研究組,由主管醫(yī)生和責(zé)任護士依據(jù)患者個體情況共同決策治療、護理、健康教育等整體目標(biāo),明確護理級別和雙方的具體任務(wù).結(jié)果 研究組婦產(chǎn)科護理質(zhì)量評分、患者滿意率顯著高于常規(guī)組（均P＜0.01）,護理級別與病情匹配率亦高于常規(guī)組.結(jié)論 婦產(chǎn)科應(yīng)用醫(yī)護合作決策護理級別,可準(zhǔn)確界定患者護理級別,提高護理工作質(zhì)量和患者滿意度.

Abstr：Objective to explore the effect of doctors and nurses determining the gradings of nursing care for patients admitted to the department of obstetrics and gynecology. Methods The 2nd unit of the department of obstetrics and gynecology was assigned into the routine group, in which patientsrnursing care levels were determined according to diseases conditions and the standards of grading system of nursing care by doctors alone, while the 1st unit was assigned into the research group, in which patients＇nursing care levels were determined by doctors and nurses together. In the research group, doctors and nurses made a joint decision about the goals of therapy, nursing care, health education, identified the grades of nursing care patients should receive, and the specific tasks of the two parties. Results The quality of nursing care, and patient satisfaction in the research group were significantly higher than in the routine group（P〈0.01 for both）. The gradings of nursing care in the research group matched their diseases conditions more than in the routine group. Conclusion The collaborative effort of doctors and nurses in determining the gradings of nursing care is able to correctly identify the nursing levels patients are supposed to receive, and improve the quality of nursing care, and pa- tient satisfaction.
