【摘要】: 目的研究國(guó)人脛骨平臺(tái)形態(tài),得到國(guó)人膝關(guān)節(jié)脛骨平臺(tái)截骨面相關(guān)解剖參數(shù),探討此方法對(duì)假體設(shè)計(jì)的指導(dǎo)意義。 方法選擇2005年3月至2006年8月在上海市第六人民醫(yī)院關(guān)節(jié)外科住院治療的168例206個(gè)病態(tài)膝關(guān)節(jié),均為初次行全膝關(guān)節(jié)置換術(shù),術(shù)中術(shù)后對(duì)脛骨平臺(tái)直接進(jìn)行相關(guān)參數(shù)測(cè)量。對(duì)所測(cè)得的數(shù)據(jù)輸入SARS 7.0統(tǒng)計(jì)學(xué)軟件進(jìn)行結(jié)果分析,求出均值及標(biāo)準(zhǔn)差,并給出脛骨平臺(tái)內(nèi)外側(cè)前后徑及脛骨平臺(tái)寬的相關(guān)系數(shù),給出回歸方程。 結(jié)果所有206膝都進(jìn)入結(jié)果分析。確立了脛骨平臺(tái)截骨后各個(gè)參數(shù)之間的比例關(guān)系,結(jié)果按患者的身高分為A組,B組,C組,得出回歸方程。三組各參數(shù)在統(tǒng)計(jì)學(xué)上無(wú)性別差異(P0.05),在統(tǒng)計(jì)學(xué)上也無(wú)左右膝差異(P0.05)。A組,脛骨內(nèi)側(cè)平臺(tái)前后徑DMTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.8246,回歸方程:DMTP=-11.8702+0.8341WTP;脛骨外側(cè)平臺(tái)前后徑DLTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.8478,回歸方程:DLTP=-10.6513+0.7197WTP。B組,脛骨內(nèi)側(cè)平臺(tái)前后徑DMTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.8062,回歸方程:DMTP=-11.9522+0.8341WTP;脛骨外側(cè)平臺(tái)前后徑DLTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.8078,回歸方程:DLTP=-11.6233+0.7992WTP。C組,脛骨內(nèi)側(cè)平臺(tái)前后徑DMTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.7234,回歸方程:DMTP=-12.2436+0.8298WTP;脛骨外側(cè)平臺(tái)前后徑DLTP與脛骨平臺(tái)寬WTP存在線性關(guān)系(P0.05),相關(guān)系數(shù):0.7529,回歸方程:DLTP=-10.7347+0.7289WTP。脛骨截骨面橫徑/前后徑比值(ML/AP):A組1.62-1.81,B組1.56-1.77,C組1.52-1.74,ML/AP均大于同尺寸進(jìn)口假體比例。 結(jié)論本研究測(cè)量并統(tǒng)計(jì)了國(guó)人病態(tài)膝關(guān)節(jié)脛骨平臺(tái)的形態(tài)學(xué)尺寸,得出國(guó)人脛骨平臺(tái)截骨面的橫徑與前后徑比值大于西方人,在矮小的國(guó)人女性中更明顯。本文建立了國(guó)人病態(tài)膝關(guān)節(jié)脛骨平臺(tái)相關(guān)參數(shù)指標(biāo)參考值,為人工膝關(guān)節(jié)假體的設(shè)計(jì)、制作提供了更加符合國(guó)人解剖結(jié)構(gòu)的科學(xué)依據(jù)和參考價(jià)值。
[Abstract]:Objective to study the shape of Chinese tibial plateau and to obtain the anatomical parameters related to the osteotomy of the tibial plateau of Chinese knee joint, and to explore the guiding significance of this method in the design of the prosthesis. Methods from March 2005 to August 2006, 206 diseased knee joints of 168 patients who were hospitalized in arthrosurgery, Shanghai sixth people's Hospital, were all performed total knee arthroplasty for the first time. The tibial plateau parameters were measured directly during and after operation. The results of the statistical software SARS-7.0 were analyzed, the mean value and standard deviation were calculated, the correlation coefficients of the anteroposterior diameter and the width of the tibial plateau were given, and the regression equation was given. Results all 206 knees entered the result analysis. The proportional relationship between the parameters after tibial plateau osteotomy was established. Results according to the height of the patients, the patients were divided into group A and group B, and the regression equation was obtained. There was no statistical difference in each parameter between the three groups (P 0.05), and there was no statistical difference in the left and right knees (P 0.05). There was a linear relationship between the anteroposterior diameters of the medial tibial plateau and the width of the tibial plateau (WTP). The correlation coefficient was: 0: 0.8246, the regression equation was: DMTP- 11.8702 0.8341WTP, and the linear relationship between the anteroposterior diameters of the tibial platform and the width WTP of the tibial plateau was P0.05N, the correlation coefficient was 0.0.8478and the regression equation was: DLTP-10.6513 0.7197WTP.B. There was a linear relationship between the anteroposterior diameters of the medial tibial plateau (DMTP) and the width of the tibial plateau (WTP). The correlation coefficient was: 0: 0.8062, the regression equation was: DMTP- 11.9522 0.8341WTP; the linear relationship between the anteroposterior diameters of the tibial platform and the width of the tibial plateau was P0.05N, the correlation coefficient was 0.8078, and the regression equation was: DLTP-11.6233 0.7992WTP.C. There was a linear relationship between the anteroposterior diameters of the tibial plateau and the width of the tibial plateau WTP, the correlation coefficient was: 0: 0.7234, the regression equation was: DMTP- 12.2436 0.8298 WTP, and the linear relationship between the anteroposterior diameters of the tibial plateau and the width of the tibial plateau was P0.05, the correlation coefficient was 0.75299.The regression equation was DLTP- 10.7347 0.7289 WTP. The ratio of transverse diameter to anteroposterior diameter of tibial osteotomy was greater in group A than that in group A (1.62-1.81) and group B (1.56-1.77) and group C (1.52-1.74) was larger than that of imported prosthesis of the same size. Conclusion this study measured and counted the morphological dimensions of the tibial plateau of the diseased knee in Chinese. The ratio of transverse diameter to anteroposterior diameter of the osteotomy of the Chinese tibial plateau was higher than that of the westerners, especially in the short Chinese women. In this paper, the reference values of the parameters related to the tibial plateau of the diseased Chinese knee joint are established, which provide the scientific basis and reference value for the design and manufacture of the artificial knee joint prosthesis more in accordance with the anatomical structure of the Chinese people.
