【摘要】:背景與目的腸易激綜合征(abdominal withdrawal reflex,IBS)是一組以腹痛或腹部不適同時(shí)伴有排便習(xí)慣及大便性狀改變?yōu)榘Y狀的腸道功能性疾病。臨床上較為常見,人群流行病學(xué)調(diào)查顯示IBS的發(fā)病率約為5%~10%[1],歐美等發(fā)達(dá)國(guó)家發(fā)病率較高,國(guó)內(nèi)以北京、廣州、上海、浙江等地高發(fā)。該病嚴(yán)重影響患者的生活質(zhì)量,但其發(fā)病機(jī)制尚未完全清楚。目前認(rèn)為IBS的發(fā)病可能與內(nèi)臟感覺(jué)異常、腸道動(dòng)力異常、感染、神經(jīng)-內(nèi)分泌、遺傳等多種因素有關(guān),其中內(nèi)臟感覺(jué)異常被認(rèn)為是IBS主要的病理生理學(xué)基礎(chǔ)。在內(nèi)臟感知過(guò)程中,長(zhǎng)期以來(lái)人們推測(cè)來(lái)自胃腸道的疼痛感覺(jué)是由脊髓傳入纖維所介導(dǎo)的[2],疼痛信號(hào)起源于初級(jí)傷害性受體外周末梢,一種位于脊髓背根神經(jīng)節(jié)(dorsal root ganglion neurons, DRG)上的特殊神經(jīng)元,傷害性感受器能通過(guò)特殊受體如瞬時(shí)感受器電位香草素受體1(transient receptor potential vanilloid type-1,TRPV1)和/或P2X嘌呤能受體(P2X purinoreceptors,P2XRs)將傷害性刺激轉(zhuǎn)變?yōu)橐环N電信號(hào),并將這種信號(hào)由外周向中樞神經(jīng)傳遞,引起相應(yīng)的反射,調(diào)節(jié)內(nèi)臟活動(dòng)。但目前,TRPV1和P2XRs對(duì)內(nèi)臟感覺(jué)調(diào)節(jié)的確切作用機(jī)制仍不清楚。因此在本研究中,我們采用大鼠乙酸造模制備內(nèi)臟高敏感模型,用1,1'-二(十八烷基)-3,3,3',3'-四甲基吲哚羰基花青高氯酸鹽(DiI)逆行示蹤標(biāo)記結(jié)腸特異背根神經(jīng)節(jié)神經(jīng)元,探討結(jié)腸特異背根神經(jīng)節(jié)神經(jīng)元的興奮性及TRPV1、P2X3受體表達(dá)情況,為研究IBS相關(guān)性腹痛提供新的治療靶點(diǎn)。 方法出生10天的大鼠,結(jié)腸內(nèi)給予0.5%乙酸(acetic acid,,AA)溶液制備內(nèi)臟高敏感大鼠模型(AA組),對(duì)照組(NS組)給予同等量0.9%生理鹽水(normalsaline,NS)。對(duì)成年后的大鼠進(jìn)行結(jié)直腸擴(kuò)張(CRD),主要用腹部回撤反射(AWR)值來(lái)評(píng)估內(nèi)臟敏感性。在結(jié)腸壁上注射DiI逆行標(biāo)記的DRG神經(jīng)元為結(jié)腸特異DRG神經(jīng)元,酶解分離神經(jīng)節(jié)成單細(xì)胞后,熒光顯微鏡下觀察細(xì)胞形態(tài)并計(jì)數(shù)。采用全細(xì)胞膜片鉗技術(shù)記錄神經(jīng)元的閾電位和動(dòng)作電位頻率來(lái)反映結(jié)腸特異DRG神經(jīng)元的興奮性。用免疫熒光技術(shù)觀察結(jié)腸特異背根神經(jīng)節(jié)神經(jīng)元上TRPV1、P2X3受體表達(dá)情況,并分別計(jì)算AA組AWR評(píng)分值與結(jié)腸特異DRG神經(jīng)元上TRPV1受體陽(yáng)性率、動(dòng)作電位頻率、閾電流值的相關(guān)性,與結(jié)腸特異DRG神經(jīng)元上P2X3受體陽(yáng)性率、動(dòng)作電位頻率、閾電流值的相關(guān)性。 結(jié)果 1. AA組大鼠在結(jié)腸乙酸灌注后出現(xiàn)排便異常,在每日進(jìn)行刺激時(shí),大鼠表現(xiàn)比較煩躁、畏懼和嘶叫,隨著天數(shù)增加,出現(xiàn)易激惹和好斗傾向; 2.與NS組相比,AA組大鼠分別在同一結(jié)直腸擴(kuò)張壓力(5.332、7.998、10.664kPa)下AWR值明顯增高(P0.05); 3. AA組結(jié)腸特異DRG神經(jīng)元細(xì)胞直徑多分布于為35~45μm,NS組為25~35μm; 4. AA組神經(jīng)元閾電流低于NS組(P0.05),2倍閾電流刺激下,引發(fā)出的AA組動(dòng)作電位頻率高于NS組(P0.05)。 5. AA組結(jié)腸特異DRG神經(jīng)元上TRPV1和P2X3受體免疫陽(yáng)性率較正常組明顯增加(P0.01). 6. AA組AWR評(píng)分值在不同壓力結(jié)直腸擴(kuò)張下均與結(jié)腸特異DRG神經(jīng)元上TRPV1受體陽(yáng)性率(P0.01)、動(dòng)作電位頻率(P0.01)呈正相關(guān)、與閾電流值(P0.05)呈負(fù)相關(guān);與結(jié)腸特異DRG神經(jīng)元上P2X3受體陽(yáng)性率(P0.01)、動(dòng)作電位頻率(P0.01)呈正相關(guān)、與閾電流值(P0.05)呈負(fù)相關(guān)。 結(jié)論 1.經(jīng)乙酸結(jié)腸灌注后的大鼠內(nèi)臟敏感性增高,內(nèi)臟高敏感大鼠模型建立。 2.內(nèi)臟高敏感大鼠結(jié)腸特異DRG神經(jīng)元細(xì)胞增大且興奮性增加。 3.內(nèi)臟高敏感大鼠結(jié)腸特異DRG神經(jīng)元細(xì)胞上TRPV1和P2X3受體表達(dá)上調(diào)且內(nèi)臟敏感性的高低與DRG神經(jīng)元興奮性及TRPV1和P2X3受體表達(dá)量有關(guān)。
[Abstract]:BACKGROUND & OBJECTIVE : The pathogenesis of irritable bowel syndrome ( IBS ) is a group of intestinal functional diseases with abdominal pain or abdominal discomfort accompanied by bowel habits and stool traits .

Methods The rats with 10 days of birth , 0.5 % acetic acid ( AA ) solution were given in the colon to prepare the visceral highly sensitive rat model ( AA group ) , and the control group ( NS group ) was given the equivalent amount of 0.9 % normal saline ( NS ) . In order to assess the sensitivity of visceral sensitivity to adult rat dorsal root ganglion neurons , we observed the expression of TRpv1 and P2X3 receptors on the dorsal root ganglion neurons of the colon by using the technique of immunofluorescence technique . The correlation between the AWR score and the positive rate , the frequency of action potential and the threshold current of the specific DRG neurons in the colon were observed .

Results

1 . In AA group , the rats were abnormal after acetic acid infusion , and when the rats were stimulated daily , the rats were more irritable , afraid and brawl , with the increase of the number of days , the rats appeared to be irritable and aggressive ;


2 . Compared with NS group , the AWR value of AA rats increased significantly ( P0.05 ) at the same colorectal distension pressure ( 5.332 , 7.998 , 10.664kPa ) , respectively .


3 . The diameter of specific DRG neurons in AA group was 35 锝
