2型糖尿病初診患者肝臟脂肪含量的相關(guān)因素

【摘要】:目的探討伴有非酒精性脂肪肝(NAFLD)的2型糖尿病患者肝臟脂肪含量與脂質(zhì)代謝、胰島素抵抗和胰島β細(xì)胞功能的關(guān)系。方法采用3.0T質(zhì)子磁共振波譜(1H-MRS)檢測31例伴NAFLD的初診2型糖尿病患者(觀察組)肝臟脂肪含量,同時測量人體學(xué)參數(shù)和血清生化指標(biāo),口服75 g OGTT和胰島素、C肽釋放試驗,用HOMA2評估胰島素抵抗、β細(xì)胞功能,與健康體檢證實血糖處于正常范圍人員(對照組)進(jìn)行比較,分析肝臟脂肪含量與脂質(zhì)代謝、胰島素抵抗和胰島β細(xì)胞功能的關(guān)系。結(jié)果觀察組BMI、WHR、ALT、TC、HDL-C、FPG、2h PG、空腹C肽、Hb A1C、HOMA2-IR、HOMA2-%B、HOMA2-%S及LFC與對照組比較顯著升高(P0.05),TG、FINS兩組間差異無統(tǒng)計學(xué)意義(P0.05)。多元逐步回歸分析顯示,BMI增加和HOMA2-IR升高是肝臟脂肪含量升高的獨立危險因素。結(jié)論肝臟脂肪含量與肥胖、肝功能、胰島素抵抗、胰島β細(xì)胞功能相關(guān),其中肥胖與胰島素抵抗對肝臟脂肪含量影響最大。
[Abstract]:Objective to investigate the relationship between hepatic fat content, lipid metabolism, insulin resistance and islet 尾 cell function in type 2 diabetic patients with non alcoholic fatty liver disease (NAF LDD). Methods the liver fat levels in 31 newly diagnosed type 2 diabetic patients with NAFLD were measured by 3.0T proton magnetic resonance spectroscopy (1H-MRS). The anthropometric parameters and serum biochemical parameters were also measured. Oral administration of 75 g OGTT and insulin C peptide release test were performed. Insulin resistance, 尾 cell function were evaluated by HOMA2, and compared with those of normal blood glucose group (control group). The relationship between liver fat content and lipid metabolism, insulin resistance and islet 尾 cell function was analyzed. Results compared with the control group, there was no significant difference between the observation group and the control group in the two groups (P 0.05). The fasting C-peptide HbA1CU HOMA2-IRN HOMA2-S and LFC were significantly higher than those of the control group (P 0.05), and there was no significant difference between the two groups (P 0.05). Multiple stepwise regression analysis showed that the increase of HOMA2-IR and BMI were independent risk factors for the increase of liver fat content. Conclusion liver fat content is correlated with obesity, liver function, insulin resistance and islet 尾 -cell function, among which obesity and insulin resistance have the greatest influence on liver fat content.
