【摘要】：目的：回顧性分析我院5年來重癥急性胰腺炎患者臨床資料，找出同合并腹腔感染相關的易感因素，，并行細菌培養(yǎng)及藥物敏感性分析，以便更好進行診治與預防。 方法：收集280例重癥急性胰腺炎患者臨床資料，分析合并感染與非合并感染組間有差異性指標并行Logistic回歸分析，探討其同重癥急性胰腺炎合并腹腔感染的關系。收集重癥急性胰腺炎合并腹腔感染腹水或壞死組織標本，并行細菌培養(yǎng)及藥物敏感性分析。 結果：器官功能衰竭數，腹腔積液，血鈣，腸功能障礙是重癥急性胰腺炎合并腹腔感染的易感因素。目前主要致病菌仍為革蘭氏陰性菌。排名前兩位的革蘭氏陰性菌為大腸埃希菌，銅綠假單胞菌；排名前兩位的革蘭氏陽性菌為腸球菌，金黃色葡萄球菌。兩類常見致病菌均對常用抗生素普遍不敏感。 結論：臨床上早期SAP合并腹腔感染的治療需針對其易感因素以及常見致病菌譜，藥物敏感性問題采取相應治療措施，以增加臨床治療效果。
[Abstract]:Objective: to retrospectively analyze the clinical data of patients with severe acute pancreatitis (SAP) in our hospital in recent 5 years, and find out the susceptible factors associated with abdominal infection, bacteria culture and drug sensitivity analysis so as to make better diagnosis, treatment and prevention. Methods: the clinical data of 280 patients with severe acute pancreatitis were collected, and the relationship between the two groups was analyzed by Logistic regression analysis. Ascites or necrotic tissue specimens of severe acute pancreatitis complicated with abdominal infection were collected for bacterial culture and drug sensitivity analysis. Results: the number of organ failure, peritoneal effusion, blood calcium and intestinal dysfunction were the susceptible factors of severe acute pancreatitis complicated with abdominal infection. At present, the main pathogenic bacteria are still Gram-negative bacteria. The first two Gram-negative bacteria were Escherichia coli, Pseudomonas aeruginosa, and the first two Gram-positive bacteria were Enterococcus and Staphylococcus aureus. The two common pathogens are generally insensitive to common antibiotics. Conclusion: the clinical treatment of SAP with abdominal infection should be based on its susceptible factors, common pathogenic bacteria spectrum and drug sensitivity, so as to increase the effect of clinical treatment.
