【摘要】：目的 探討內(nèi)鏡下食管靜脈曲張?zhí)自�術(shù)(EVL)后門(mén)靜脈血流動(dòng)力學(xué)的變化,為臨床防治肝硬化并食管靜脈曲張破裂出血(EVB)提供指導(dǎo)。方法 選取64例肝硬化并食管靜脈重度曲張破裂出血的患者作為研究對(duì)象,行內(nèi)鏡下食管靜脈曲張?zhí)自�術(shù),套扎前后用彩色多普勒超聲對(duì)門(mén)靜脈及其屬支的血管管徑、血流速度、血流量進(jìn)行分析與比較。并對(duì)研究對(duì)象進(jìn)行隨訪1年,考察行術(shù)后患者再出血及死亡病例的情況。結(jié)果 EVL近期止血成功率為96.9%,1年內(nèi)再出血率為9.7%,術(shù)后患者門(mén)靜脈和脾靜脈的血流速度與血流量明顯增加(P0.05),胃左靜脈血流由離肝血流轉(zhuǎn)變?yōu)橄蚋窝�流。結(jié)論 EVL是治療和預(yù)防EVB安全有效的方法;EVL后門(mén)靜脈和脾靜脈的血流速度、血流量增加,肝臟的血液供應(yīng)增大。
[Abstract]:Objective to investigate the changes of portal vein hemodynamics after endoscopic esophageal variceal ligation (EVLL), and to provide guidance for clinical prevention and treatment of cirrhosis with esophageal variceal hemorrhage. Methods Sixty-four patients with cirrhosis and severe esophageal variceal bleeding were selected as study subjects. Endoscopic esophageal variceal ligation was performed. The diameter of portal vein and its branches was measured by color Doppler ultrasound before and after ligation. Blood flow velocity and blood flow were analyzed and compared. The patients were followed up for 1 year to investigate the postoperative bleeding and death. Results the recent success rate of hemostasis in EVL was 96.9 and the rate of rebleeding within one year was 9.7. The blood flow velocity and blood flow of portal vein and splenic vein increased significantly (P 0.05), and the blood flow of left gastric vein changed from free liver flow to liver flow. Conclusion EVL is a safe and effective method for the treatment and prevention of EVB.
