【摘要】：目的：本研究旨在通過(guò)觀察不同疾病胃粘膜中粘蛋白1(Mucinl, MUC1)、粘蛋白2(Mucin2, MUC2)的表達(dá)情況,初步探討MUC1、MUC2表達(dá)與胃粘膜癌變過(guò)程及MUC1、MUC2表達(dá)與幽門(mén)螺桿菌(Helicobacter pylori, HP)感染的相關(guān)性。方法：隨機(jī)選取2011年11月至2012年4月就診于青島市市立醫(yī)院,病理診斷為非萎縮性胃炎的患者30例、腸上皮化生患者30例,胃癌患者48例,用免疫組織化學(xué)方法檢測(cè)所有患者胃粘膜中MUC1和MUC2的表達(dá),并同時(shí)探討它們與HP的相關(guān)性。 結(jié)果：(1)非萎縮性胃炎組、腸上皮化生組、胃癌組胃粘膜組織中MUC1的陽(yáng)性表達(dá)率分別為93.33%、86.67%、79.17%,呈逐漸降低的趨勢(shì),但三組間MUC1的陽(yáng)性表達(dá)率兩兩比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P＞O.05)。MUC2在非萎縮性胃炎組胃粘膜組織中不表達(dá),在腸上皮化生組及胃癌組胃粘膜組織中陽(yáng)性表達(dá)率分別為83.33%、31.25%,且兩組間差異有統(tǒng)計(jì)學(xué)意義(P＞0.05)。(2)HP陽(yáng)性胃粘膜較HP陰性胃粘膜中MUC1和MUC2的陽(yáng)性表達(dá)率均低,但差異無(wú)統(tǒng)計(jì)學(xué)意義P分別為0.070、0.953,P＞O.05)。 結(jié)論：本組研究發(fā)現(xiàn)從非萎縮性胃炎、腸上皮化生到胃癌組織,MUC1表達(dá)呈逐漸下調(diào)的趨勢(shì),而MUC2僅在腸上皮化生及胃癌組織中表達(dá),提示這種現(xiàn)象可能與胃癌的發(fā)生有關(guān)。HP感染可能與胃粘膜MUC1、MUC2的陽(yáng)性表達(dá)率下降有關(guān),但其相關(guān)性需要進(jìn)一步研究探討。
[Abstract]:Objective : To investigate the relationship between MUC1 , MUC2 expression and gastric mucosal carcinogenesis , MUC1 , MUC2 expression and Helicobacter pylori ( HP ) infection by observing the expression of mucin 1 ( Mucinl , MUC1 ) and Mucin 2 ( MUC2 ) in gastric mucosa of different diseases . Methods : From November 2011 to April 2012 , 30 patients with non - atrophic gastritis , 30 patients with non - atrophic gastritis , 48 patients with gastric cancer were randomly selected , and the expression of MUC1 and MUC2 in gastric mucosa of all patients was detected by immunohistochemistry .

Results : ( 1 ) The positive rates of MUC1 were 93.33 % , 86.67 % and 79.17 % in non - atrophic gastritis group , intestinal epithelium group and gastric cancer group respectively . There was no significant difference between the two groups ( P > 0.05 ) .

Conclusion : The expression of MUC1 is gradually down - regulated from non - atrophic gastritis and intestinal epithelium to gastric cancer . MUC2 is only expressed in intestinal epithelium and gastric cancer tissues , suggesting that this phenomenon may be related to the occurrence of gastric cancer . HP infection may be related to the decrease of MUC1 and MUC2 in gastric mucosa , but its correlation needs further study .
