【摘要】：目的探討兒童急性淋巴細胞白血病(ALL)時血小板活化狀態(tài)及功能的變化。方法研究對象均來自西南醫(yī)科大學附屬醫(yī)院2012年6月至2013年11月收治的住院患兒,應(yīng)用全自動血細胞分析儀和流式細胞術(shù)(FCM)微量全血法檢測ALL患兒(ALL組)、ALL誘導緩解治療達完全緩解患兒(ALL-CR1組)以及外科擇期手術(shù)患兒(正常組)的血小板參數(shù)、網(wǎng)織血小板百分率(IPF%)、網(wǎng)織血小板絕對值(IPC)、顆粒膜糖蛋白(CD62p)、PAC-1。結(jié)果二磷酸腺苷(ADP)激活前,ALL組CD62p、PAC-1表達高于正常組(P0.05),ALL-CR1組PAC-1表達高于正常組(P0.05),低于ALL組(P0.05);ADP激活后,ALL組CD62p、PAC-1表達低于正常組(P0.05),ALL-CR1組PAC-1表達低于正常組(P0.05),高于ALL組(P0.05);ALL組血小板計數(shù)(PLT)、血小板壓積(PCT)、血小板平均體積(MPV)均低于正常組和ALL-CR1組(P0.05);ALL組IPF%高于正常組和ALL-CR1組(P0.05),IPC低于正常組和ALL-CR1組(P0.05)。結(jié)論ALL初診患兒存在血小板活化,同時伴有血小板功能異常。血小板參數(shù)及血小板膜糖蛋白可作為判斷ALL療效的有效指標。
[Abstract]:Objective to investigate the changes of platelet activation and function in children with acute lymphocytic leukemia (ALL). Methods the children with ALL were detected by automatic blood cell analyzer and flow cytometry (FCM) microblood method from June 2012 to November 2013 in the affiliated Hospital of Southwest Medical University (ALL group). Platelet parameters, percentage of reticulated platelets (IPF%) and absolute value of reticular platelet membrane glycoprotein (CD62p) in children with complete remission by ALL induced remission (ALL-CR1 group) and surgical elective surgery (normal group). PAC-1. Results before adenosine diphosphate (ADP) activation, the expression of CD62p,PAC-1 in ALL group was higher than that in normal group (P 0.05), and the expression of PAC-1 in ALL-CR1 group was higher than that in normal group (P 0.05), which was lower than that in ALL group (P 0.05). After ADP activation, the expression of CD62p,PAC-1 in ALL group was lower than that in normal group (P 0.05), and the expression of PAC-1 in ALL-CR1 group was lower than that in normal group (P 0.05), which was higher than that in ALL group (P 0.05). The mean platelet volume of (PLT), platelet volume (PCT), in ALL group was lower than that in normal group and ALL-CR1 group (P 0.05). IPF% in ALL group was higher than that in normal group and ALL-CR1 group (P 0.05), IPC was lower than that in normal group and ALL-CR1 group (P 0.05). Conclusion Platelet activation and abnormal platelet function are found in the newly diagnosed children with ALL. Platelet parameters and platelet membrane glycoprotein can be used as effective indexes to judge the curative effect of ALL.
