【摘要】：目的通過研究影響尿道下裂手術(shù)成功率的關(guān)鍵性因素,制定個體化的手術(shù)方案,提高手術(shù)成功率。方法 回顧性分析廣西醫(yī)科大學(xué)第一附屬醫(yī)院小兒外科同一手術(shù)組在2013年1月至2015年6月采用各種尿道成形術(shù)治療的尿道下裂166例。統(tǒng)計手術(shù)成功率及并發(fā)癥,對影響手術(shù)成功率的相關(guān)因素進(jìn)行單因素卡方分析和多因素邏輯回歸分析。結(jié)果全組166例手術(shù)成功率為87.95%,并發(fā)癥發(fā)生率為12.05%。其中首次手術(shù)一期尿道成形組75例,手術(shù)成功率為90.67%,并發(fā)癥發(fā)生率為9.33%。首次手術(shù)分期尿道成形組28例,手術(shù)成功率89.29%,并發(fā)癥發(fā)生率10.71%。再次手術(shù)尿道成形組63例,手術(shù)成功率85.71%,并發(fā)癥發(fā)生率為14.29%。統(tǒng)計分析結(jié)果顯示：本組尿道下裂手術(shù)成功率與患者陰莖頭直徑、陰莖伸直后尿道外口位置及術(shù)后切口感染等影響因素顯著相關(guān),與手術(shù)年齡、陰莖下曲程度、尿道缺損長度及手術(shù)方法等因素?zé)o顯著相關(guān)。結(jié)論影響尿道下裂手術(shù)成功率的因素眾多,對于同一手術(shù)者而言,根據(jù)患者陰莖發(fā)育情況和具體解剖條件制定個性化的手術(shù)方案,同時加強(qiáng)圍手術(shù)期精細(xì)化管理,是提高手術(shù)成功率的最有效方法。
[Abstract]:Objective to study the key factors affecting the success rate of hypospadias and to make an individualized operation plan to improve the success rate of hypospadias. Methods 166 cases of hypospadias were treated with various urethroplasty from January 2013 to June 2015 in the same operation group of Pediatric surgery, the first affiliated Hospital of Guangxi Medical University. The success rate and complications of the operation were counted, and the related factors affecting the success rate of the operation were analyzed by univariate chi-square analysis and multivariate logical regression analysis. Results the success rate of 166 cases was 87.95% and the incidence of complications was 12.05%. Among them, 75 cases were in the first stage urethroplasty group, the success rate was 90. 67%, and the incidence of complications was 9. 33%. In the first stage urethroplasty group, the success rate was 89.29% and the incidence of complications was 10.71%. In the reoperation urethroplasty group, the success rate was 85.71% and the incidence of complications was 14.29%. The results of statistical analysis showed that the success rate of hypospadias was significantly correlated with the diameter of penile head, the position of external urethral orifice after penis straightening and incision infection after operation, which was related to the age of operation and the degree of inferior curvature of penis. There was no significant correlation between the length of urethral defect and the method of operation. Conclusion there are many factors that affect the success rate of hypospadias surgery. For the same operation, the individualized operation plan should be made according to the penile development and specific anatomical conditions of the patients, and the fine management during perioperative period should be strengthened at the same time. It is the most effective method to improve the success rate of operation.
