【摘要】:研究背景隨著我國(guó)醫(yī)學(xué)模式的轉(zhuǎn)變和醫(yī)療市場(chǎng)的激烈競(jìng)爭(zhēng),人們對(duì)健康的需求和維權(quán)的意識(shí)不斷提高,對(duì)護(hù)理工作也提出了更高的要求。職業(yè)認(rèn)同是職業(yè)人在工作中馳騁的重要心理基礎(chǔ),是發(fā)展的內(nèi)在激勵(lì)因素。護(hù)理人員職業(yè)認(rèn)同感可以影響護(hù)士對(duì)工作的積極性和滿意度,從而影響護(hù)理質(zhì)量。目前,我國(guó)護(hù)士短缺,較高的護(hù)士離職率是引起護(hù)士短缺的主要原因之一。離職意愿是離職最有效的預(yù)測(cè)指標(biāo),最終可能導(dǎo)致實(shí)際離職的發(fā)生。兒科由于其服務(wù)對(duì)象的特殊性和護(hù)理技能的高要求,在工作中要付出更多的精力和時(shí)間。兒科護(hù)理人員不僅要處理好與患兒及家屬的關(guān)系,還要處理好與同事間的關(guān)系,同時(shí)暴力事件還會(huì)嚴(yán)重威脅護(hù)士的身心健康,導(dǎo)致護(hù)士職業(yè)認(rèn)同降低和離職意愿增強(qiáng)。為探索泰安市兒科護(hù)理人員職業(yè)認(rèn)同與離職意愿現(xiàn)狀及相關(guān)因素,對(duì)泰安市兒科護(hù)理人員進(jìn)行了現(xiàn)況調(diào)查。研究目的本研究旨在通過(guò)對(duì)泰安市兒科護(hù)理人員進(jìn)行問(wèn)卷調(diào)查,了解兒科護(hù)理人員的職業(yè)認(rèn)同現(xiàn)況及離職意愿現(xiàn)況,進(jìn)一步探索影響兒科護(hù)理人員職業(yè)認(rèn)同及離職意愿的相關(guān)因素,找出職業(yè)認(rèn)同及離職意愿的相關(guān)性,從而提高泰安市兒科護(hù)理人員的職業(yè)認(rèn)同,降低其離職意愿,穩(wěn)定兒科護(hù)理隊(duì)伍。研究對(duì)象與方法本研究以泰安市兒科護(hù)理人員為研究對(duì)象,在取得醫(yī)院護(hù)理部領(lǐng)導(dǎo)的同意和科護(hù)士長(zhǎng)的支持下,對(duì)泰安市3所三級(jí)甲等綜合醫(yī)院、2所三級(jí)甲等?漆t(yī)院(中醫(yī)藥系列和兒童系列)及1所二級(jí)甲等綜合醫(yī)院共6家醫(yī)院的兒科護(hù)理人員(每家醫(yī)院兒科護(hù)理人員人數(shù)在20人以上)發(fā)放調(diào)查問(wèn)卷。共發(fā)放問(wèn)卷460份,回收有效問(wèn)卷442份,有效回收率為96.1%。主要調(diào)查指標(biāo):兒科護(hù)理人員的人口社會(huì)學(xué)特征、職業(yè)認(rèn)同水平及其離職意愿狀況。數(shù)據(jù)處理采用Excel 2007錄入數(shù)據(jù),運(yùn)用SPSS 20.0對(duì)資料進(jìn)行統(tǒng)計(jì)學(xué)分析。主要結(jié)果一、職業(yè)認(rèn)同狀況及相關(guān)因素研究對(duì)象職業(yè)認(rèn)同總分為97.45±16.37,得分率為64.97%;職業(yè)認(rèn)同5個(gè)維度得分率由高到低為職業(yè)社會(huì)支持68.77%,職業(yè)挫折應(yīng)對(duì)68.03%,職業(yè)自我反思66.80%,職業(yè)社交技能62.30%,職業(yè)認(rèn)知評(píng)價(jià)61.35%?剖、工作年限、聘任方式、醫(yī)院等級(jí)和工作環(huán)境滿意度5個(gè)因素具有統(tǒng)計(jì)學(xué)意義(P0.05)。二、離職意愿狀況及相關(guān)因素離職意愿總分為15.66±3.08,其中離職意愿Ⅰ指標(biāo)值61.28%,離職意愿Ⅱ最低,指標(biāo)值60.89%,離職意愿Ⅲ最高,指標(biāo)值73.56%。對(duì)兒科護(hù)理人員離職意愿總分按照等級(jí)劃分標(biāo)準(zhǔn)進(jìn)行劃分,離職意愿很低的沒(méi)有;離職意愿較低的占17.9%;離職意愿較高的占52.3%;離職意愿很高的占29.9%。聘任方式、醫(yī)院等級(jí)、職工自豪感及工作原因4個(gè)因素具有統(tǒng)計(jì)學(xué)意義(P0.05)。三、職業(yè)認(rèn)同與離職意愿的相關(guān)性職業(yè)認(rèn)同及各維度與離職意愿、離職意愿Ⅰ、離職意愿Ⅱ均成負(fù)相關(guān);職業(yè)認(rèn)知評(píng)價(jià)與離職意愿Ⅲ成負(fù)相關(guān)性;職業(yè)認(rèn)同及4個(gè)維度與離職意愿Ⅲ無(wú)相關(guān)性。結(jié)論泰安市兒科護(hù)理人員職業(yè)認(rèn)同處于中等水平。研究對(duì)象對(duì)所從事的護(hù)理本職工作的概念、意義及價(jià)值認(rèn)知不夠深入,對(duì)自身的護(hù)理人員角色的認(rèn)知和評(píng)價(jià)不高?剖摇⒐ぷ髂晗、聘任方式、醫(yī)院等級(jí)和工作環(huán)境滿意度是影響兒科護(hù)理人員職業(yè)認(rèn)同總體水平的重要因素。泰安市兒科護(hù)理人員離職意愿較強(qiáng),獲得外部其他工作可能性較大。聘任方式、醫(yī)院等級(jí)、職工自豪感及工作原因是影響兒科護(hù)理人員離職意愿的重要因素。泰安市兒科護(hù)理人員職業(yè)認(rèn)同及其5個(gè)維度越高,離職意愿越低,辭去當(dāng)前工作及尋找其他相同或不同性質(zhì)工作動(dòng)機(jī)也會(huì)降低。職業(yè)認(rèn)知評(píng)價(jià)越高,獲得外部工作的可能性越低。
[Abstract]:Background: with the medical mode of our country and the transformation of the medical market competition, awareness of health needs and rights continue to improve, but also put forward higher requirements to the nursing work. Occupation identity is an important psychological basis of human occupation gallop in the work, is the internal incentive factors. The development of nursing staff occupation. The same can affect the enthusiasm and satisfaction of nurses on the work, thus affecting the quality of nursing. At present, China's shortage of nurses, the high turnover rate of nurses is one of the main reasons causing the shortage of nurses. Turnover intention is the most effective predictor of turnover, may eventually lead to the occurrence of actual turnover. Because of the high requirements and special pediatric nursing the skills of service object, in order to spend more time and energy. Nursing staff should not only handle the relationship with the children and their families, but also deal with The relationship between colleagues, and violence but also a serious threat to the physical and mental health of nurses, to reduce the turnover intention of nurses' occupation identity and enhancement. In order to explore the nursing staff in Tai'an city of occupation identity and turnover intention status and related factors of nurses in Tai'an City pediatric for investigation. The purpose of this study is to through questionnaire survey the nursing staff in Tai'an in pediatrics, pediatric nurses to understand the occupation identity and turnover intention status, to further explore the effects of pediatric nurses' occupation identity and related factors of turnover intention, to find out the correlation between occupation identity and turnover intention, so as to improve the occupation identity of nursing staff in Tai'an city children, reduce turnover intention, pediatric nursing team stability the object of study and the method of the study on pediatric nursing personnel in Tai'an city as the research object, made in the hospital nursing department. The consent and the head nurse's support, 3 in Tai'an and three general hospitals, 2 three level of first-class hospital (traditional Chinese medicine series and the series of children) and 1 pediatric nurses in grade two general hospital a total of 6 hospitals (the number of pediatric nurses every hospital in more than 20 people) questionnaire. A total of 460 questionnaires, 442 valid questionnaires, the effective recovery rate as the main investigation index 96.1%.: pediatric nurses' demographic and sociological characteristics, occupation identity level and turnover intention status. Conclusion the nursing staff of pediatric Tai'an city occupation identity is in the medium level. The Department, working life, employment, the degree of satisfaction and working environment of the hospital is an important factor affecting the overall level of pediatric nurses' occupation identity. Tai'an City pediatric nurses' turnover intention is strong, get outside other work more likely. The appointment way, the rank of the hospital. The pride and work is an important factor to influence turnover intention of nursing staff in pediatrics. Tai'an pediatric nurses' occupation identity and the 5 dimensions of the higher, The lower the turnover intention, the lower the resignation of the current job and the other same or different nature of work motivation. The higher the occupation cognitive evaluation, the lower the possibility of obtaining external work.
