【摘要】：目的本研究旨在了解克羅恩病患者自我鼻飼的出發(fā)點(diǎn)、鼻飼期間遇到的問(wèn)題和解決措施,以及自我鼻飼對(duì)其生活和工作各方面產(chǎn)生的影響,通過(guò)探討克羅恩病患者自我鼻飼體驗(yàn),增進(jìn)醫(yī)務(wù)人員及社會(huì)大眾對(duì)該群體的認(rèn)識(shí),為其臨床干預(yù)和社會(huì)支持提供依據(jù)。方法采用詮釋性現(xiàn)象學(xué)研究方法,通過(guò)目的抽樣,選取來(lái)自杭州市多家綜合醫(yī)院的門診及病房就診的克羅恩病自我鼻飼患者為研究對(duì)象,進(jìn)行個(gè)人深度訪談。資料分析主要按照Benner分析法進(jìn)行,直至主題飽和,再無(wú)新主題產(chǎn)生為止。結(jié)果共訪談11名克羅恩病患者,訪談資料經(jīng)分析形成四個(gè)主題八個(gè)亞主題:1排斥鼻飼:恐懼插管、存在顧慮;2接受鼻飼現(xiàn)實(shí):身體最重要、力量的支撐;3鼻飼是把雙刃劍:鼻飼的困擾、鼻飼的療效;4鼻飼如家常便飯:對(duì)插管習(xí)以為常、鼻飼成為生活的一部分。結(jié)論本研究獲得了關(guān)于克羅恩病患者自我鼻飼體驗(yàn)的理解�？肆_恩病患者在自我鼻飼期間面臨諸多心理考驗(yàn),且增添了軀體不適、飲食和形象異常以及生活不便的困擾,對(duì)此,患者能夠自我調(diào)整或予以克服,最終適應(yīng)鼻飼。但并非每位患者都能達(dá)到這種適應(yīng)狀態(tài),醫(yī)務(wù)人員應(yīng)調(diào)動(dòng)醫(yī)護(hù)患及社會(huì)大眾等各方面力量,幫助患者盡快適應(yīng)"新生活""。
[Abstract]:Objective to investigate the starting point of self-feeding of Crohn's disease patients, the problems encountered during nasal feeding and their solutions, and the influence of self-feeding on their life and work. By exploring the experience of self-feeding in patients with Crohn's disease and enhancing the understanding of this group among the medical staff and the general public, we can provide the basis for its clinical intervention and social support. Methods the hermeneutic phenomenological research method was adopted. Objective to select self-feeding patients of Crohn's disease from outpatients and wards of many general hospitals in Hangzhou. Conducted personal in-depth interviews. Data analysis was mainly conducted according to the Benner analysis until the theme saturated no new topics. Results A total of 11 patients with Crohn's disease were interviewed. The interview data were analyzed into four themes, eight sub-themes: 1 rejection of nasal feeding: fear intubation, concern; (2) accept the reality of nasal feeding: the body is the most important, the support of strength; (3) nasal feeding is a double-edged sword: the puzzlement of nasal feeding and the curative effect of nasal feeding; 4 nasal feeding as usual: intubation is used to take for granted. Conclusion this study has gained an understanding of the experience of self-feeding in patients with Crohn's disease. Patients with Crohn's disease face many psychological tests during self-feeding and increase physical discomfort. Abnormal diet and image, as well as the inconvenience of life, patients can self-adjust or overcome, and finally adapt to nasal feeding, but not every patient can achieve this state of adaptation. Medical personnel should mobilize medical care and patients and the public to help patients adapt to the new life as soon as possible.
