【摘要】：目的探討老年2型糖尿病(T2DM)患者認知損害特點及影響因素。方法采用蒙特利爾認知評估量表(MOCA)、日常生活能力量表和總體衰退量表對住院治療的176例老年T2DM患者進行評測。結(jié)果 86.36%的老年T2DM患者延遲記憶受損,75.57%視空間/執(zhí)行功能受損;69.89%抽象功能受損;36.36%注意功能受損;35.23%語言功能受損;15.34%定向功能受損。單個認知域受損率為14.20%,2個13.64%,3個28.41%,4個19.32%,5個13.64%,6個7.95%。單因素分析顯示年齡、性別、文化程度、家庭月收入、飲酒、糖化血紅蛋白(Hb A1c)和低血糖史有統(tǒng)計學意義;多因素Logistic回歸顯示年齡、文化程度、Hb A1c是認知損害的影響因素(P0.05)。結(jié)論老年T2DM患者表現(xiàn)以延遲記憶、視空間/執(zhí)行功能、抽象功能等為主的多認知域聯(lián)合受損。醫(yī)務人員應關注糖尿病老年人群的認知損害,采取多種措施延緩認知功能減退。
[Abstract]:Objective to investigate the cognitive impairment characteristics and influencing factors of elderly type 2 diabetes mellitus T 2 DM. Methods the Montreal Cognitive Assessment scale (MOCA), the activity of Daily living scale (ADL) and the General decline scale (GTS) were used to evaluate 176 elderly patients with T2DM. Results in 86.36% of the elderly patients with T2DM, 75.57% of them were impaired in visual space / executive function, 69.89% in abstract function, 36.36% in attention function and 35.23% in language function and 15.34% in directional function. The damage rate of single cognitive domain was 14.20, two 13.64, three 28.41, four 19.32, five 13.64 and six 7.95. Univariate analysis showed that age, sex, education level, family monthly income, alcohol consumption, HbA1c) and history of hypoglycemia were statistically significant, and age, education level and HbA1c were the influencing factors of cognitive impairment by multivariate Logistic regression analysis. Conclusion in elderly patients with T2DM, delayed memory, visual space / executive function and abstract function are the main manifestations of multiple cognitive domain joint impairment. Medical staff should pay attention to the cognitive impairment of the elderly diabetic population and take various measures to delay cognitive impairment.
