心電圖引導(dǎo)PICC尖端定位的多中心研究

【摘要】:目的探討心電圖引導(dǎo)PICC尖端定位的準(zhǔn)確性及體外測(cè)量長(zhǎng)度對(duì)最佳置入長(zhǎng)度的預(yù)測(cè)價(jià)值。方法便利抽取了全國11個(gè)省、市、自治區(qū)的18所三級(jí)甲等醫(yī)院的743例置管患者進(jìn)行心電圖引導(dǎo)PICC尖端定位。置管到達(dá)最佳位置后,進(jìn)行胸部X線定位,觀察心電圖引導(dǎo)PICC尖端定位的準(zhǔn)確性。結(jié)果 624例患者經(jīng)心電圖引導(dǎo)PICC尖端定位后,X線胸片顯示導(dǎo)管尖端均在最佳位置。PICC尖端最佳置入長(zhǎng)度與體外測(cè)量長(zhǎng)度呈正相關(guān)(r=0.920,P0.01)。以PICC最佳置入長(zhǎng)度(Y)為因變量,以體外測(cè)量長(zhǎng)度(X)為自變量,采用逐步線性回歸,得出回歸方程為:左側(cè)貴要靜脈Y=4.290+0.897X(R~2=0.834,F=942.459,P0.001),右側(cè)貴要靜脈Y=3.097+0.922X(R~2=0.855,F=2055,P0.001)。結(jié)論對(duì)于心電圖有P波的患者,P波變化可用來指導(dǎo)置管操作和預(yù)測(cè)導(dǎo)管尖端位置,心電圖定位可作為一種替代性方法。經(jīng)貴要靜脈置入PICC時(shí),可借助線性回歸方程,預(yù)測(cè)導(dǎo)管置入長(zhǎng)度。
[Abstract]:Objective to explore the value of prediction accuracy and measuring the length of in vitro ECG guided PICC tip positioning on the optimum insertion length. A convenience sample of the country's 11 provinces, city, autonomous region 18 three hospitals and 743 cases of catheter patients ECG guided PICC catheter tip location. To reach the best position, chest X-ray the positioning accuracy, observe the ECG guided PICC tip positioning. Results 624 cases of patients with ECG guided PICC tip position, chest X-ray showed the catheter tip are in the best position of the tip of.PICC optimal insertion length and length measurement in vitro was positively correlated (r=0.920, P0.01). The PICC optimal placement length (Y) as the dependent variable, in vitro measure the length (X) as the independent variable, stepwise linear regression, the regression equation was: on the left side of the basilic vein Y=4.290+0.897X (R~2=0.834, F=942.459, P0.001), Y=3.097+0.922X R (on the right side of the basilic vein ~2=0.855, F=2055, P0.001). Conclusion for patients with P wave ECG, P wave changes can be used to guide the catheter operation and prediction of catheter tip position, ECG can be used as an alternative method. The basilic vein in PICC, with the linear regression equation, prediction of catheter insertion length.

【正文快照】: 目前,PICC在臨床得到了廣泛的應(yīng)用。一般PICC尖端應(yīng)置于上腔靜脈末端,靠近上腔靜脈與右心房入口交界處為最佳位置[1]。一直以來,X線定位法被視為PICC尖端定位的“金標(biāo)準(zhǔn)”,但一旦發(fā)現(xiàn)定位錯(cuò)誤需要重新置管,不僅耗費(fèi)時(shí)間,同時(shí)增加了輻射暴露的機(jī)會(huì),并且禁用于孕婦。心電圖引導(dǎo)P
